In order to further imProve the management of hypertension in county areas, the Chinese Pharmacists Association and the Hainan Boao County Medical Development Research Center jointly organized and compiled the County Hypertension Rational Use of Drugs and Comprehensive Management Guidelines (hereinafter referred to as the Guidelines). At the release conference, the corresponding author of the Guidelines, Professor Sun Ningling, director of the Cardiology Center of Peking University People's Hospital, introduced that the Guidelines provide county medical institutions with a scientific, standardized, and practical hypertension management Plan. This will promote the standardization and precision of hypertension prevention and treatment work, effectively improve the level of hypertension prevention and control in county areas, and bring better and more efficient medical services to a vast number of hypertensive patients in county areas.

Core Contents

The Guidelines mainly include core content such as the epidemiology and grassroots medication use status of hypertension, classification of antihypertensive drugs, medication principles and interactions, rational use of single-drug and single-tablet compound preparations of antihypertensive drugs, and comprehensive factor management of grassroots hypertension. It follows the new evidence of evidence-based medicine, and on the basis of theoriginal five classes of antihypertensive drugs, some new antihypertensive drugs have been added, more antihypertensive drugs suitable for county grassroots application have been proposed, the antihypertensive treatment principles have been clarified, and the accuracy, safety, and adaptability of drug treatment in county areas have been emphasized.

Medication Principles

Currently, grassroots medical institutions mainly based on commUnity health service organizations are the main battleground for hypertension prevention and treatment. The mastery of antihypertensive drugs by grassroots medical staff directly affects the control status of hypertension. To this end, the Guidelines give six medication principles for antihypertensive drugs:

1. Reduce risk. It is recommended to choose antihypertensive drugs with evidence support that can improve cardiovascular prognosis (Class I recommendation, Grade A evidence).
2. Long-term antihypertensive effect. Antihypertensive treatment not only needs to reach the target blood pressure, but also needs to m03cstrong>Time of administration. Most of the evidence on the benefits of hypertension treatment comes from randomized controlled trials that administer thedrug in the morning. However, recent evidence suggests that non-dipper hypertension may have a poor prognosis. A prospective, randomized, open-end clinical trial showed that compared with morning administration, evening administration of antihypertensive drugs did not bring more cardiovascular benefits. Unless there is a clear need to control nocturnal or early morning blood pressure elevation, it is not recommended to routinely administer antihypertensive drugs before bedtime (Class II recommendation, Grade A evidence).
3. Individualized treatment. Choose antihypertensive drugs that are suitable for individual patients according to the patient's comorbidities, drug efficacy, and tolerability, as well as the patient's personal wishes or long-term affordability (Class I recommendation, Grade A evidence).

Points to Note

The Guidelines suggest that clinicians should pay attention to the following points when choosing antihypertensive drugs:

1. Choose drugs according to the clinical phenotype of hypertension. The European Society of Hypertensionidelines is of great significance for improving the standardized and precise management of hypertension in county areas, and for effectively improving the level of hypertension prevention and control in county areas. It is believed that under the guidance of the Guidelines, the grassroots medical staff incounty areas will be able to better serve the vast number of hypertensive patients, effectively control blood pressure, reduce cardiovascular and cerebrovascular complications, and improve the quality of life of hypertensive patients.
