Medication Principles

Currently, grassroots medical institutions mainly based on commUnity health service organizations are the main battleground for hypertension prevention and treatment. The mastery of antihypertensive drugs by grassroots medical staff directly affects the control status of hypertension. To this end, the Guidelines give six medication principles for antihypertensive drugs:

  1. Reduce risk. It is recommended to choose antihypertensive drugs with evidence support that can improve cardiovascular prognosis (Class I recommendation, Grade A evidence).
  2. Long-term antihypertensive effect. Antihypertensive treatment not only needs to reach the target blood pressure, but also needs to m03cstrong>Time of administration. Most of the evidence on the benefits of hypertension treatment comes from randomized controlled trials that administer thedrug in the morning. However, recent evidence suggests that non-dipper hypertension may have a poor prognosis. A prospective, randomized, open-end clinical trial showed that compared with morning administration, evening administration of antihypertensive drugs did not bring more cardiovascular benefits. Unless there is a clear need to control nocturnal or early morning blood pressure elevation, it is not recommended to routinely administer antihypertensive drugs before bedtime (Class II recommendation, Grade A evidence).
  3. Individualized treatment. Choose antihypertensive drugs that are suitable for individual patients according to the patient's comorbidities, drug efficacy, and tolerability, as well as the patient's personal wishes or long-term affordability (Class I recommendation, Grade A evidence).

The Guidelines suggest that clinicians should pay attention to the following points when choosing antihypertensive drugs:

  1. Choose drugs according to the clinical phenotype of hypertension.
